TIP: PRINT THIS PAGE TO USE FOR REFERENCE as you complete the online form.

1) GO TO: http://permits.westfordma.gov

2) In the left column, CLICK THE BOX that says "Apply for a Permit without a User Name"

3) On the page that comes up, you will start filling out a form. TO ENSURE PROPER PROCESSING, ENTER THE INFORMATION IN THIS ORDER:

Property/Owner Information Block:

a) Location Type: the "Address" button should already be selected
b) Street Name: type in POWERS RD. and hit the TAB Key (this will cause a new line to appear for Street Number)
c) Street Number: type in 79 and hit TAB Key
d) City State Zip line should saY "WESTFORD MA 01886"
e) Parcel ID: Click on the drop-down menu and select (1) 005 0003 0000 (some additional info will come up.)

Applicant Information Section:

f) In the Lower left corner block, ENTER YOUR COMPANY'S INFORMATION (business name, address, phone, email)

Permits/Licenses Section:

g) In the right hand column, second box down (Board of Health box) scroll down and CLICK THE CHECKBOX for "Temporary Food Permit"

h) CLICK the "Continue" button at the bottom of the page. It may appear in the left hand column on your screen.

4) This will take you to a new page. FILL OUT THE ONLINE FORM and click Submit. This will bring you to an acknowledgement page.

NOTE: The wording on the acknowledgement page might report that the permit fee is $0 and say "No Outstanding Documentation Needed". This is incorrect. SEE #7 BELOW.

5) YOU MUST CLICK THE PRINT BUTTON to print out a copy of your completed form.

6) On the print-out, write in your Federal ID#, and Sign & Date the bottom of the form.

7) Mail the form to the Westford Board of Health along with:

$30 permit fee
A copy of your permit from your town
A copy of your Serve-Safe Certificate

The mailing address is at the top of the printed form.

IMPORTANT: Your application will not be reviewed until payment is recieved. YOUR FORM AND PAYMENT MUST BE RECEIVED BY THE BOARD OF HEALTH AT LEAST 2 WEEKS PRIOR TO THE EVENT.
